1. Scope of Services

I provide clinical services related to podiatry. All treatments are delivered by myself, a qualified professional in accordance with the Royal college of Podiatry and Health & care professional council standards.

2. Appointments

• Appointments are to be booked via telephone and paid in full at time of appointment.

• Cancellations or rescheduling require at least 72 hours notice.

• For any reason, missed appointments will incur a fee of £15 which is to be paid at your next appointment.

3. Patient Responsibilities

Patients are expected to:

• Provide accurate medical history and information.

• Inform myself of any changes in health or medication.

4. Fees and Payments

• Fees for services are outlined in our pricing schedule.

• Payment is due at the time of service unless otherwise agreed.

• I accept cash, credit cards and BACS.

5. Confidentiality

All patient information is kept confidential and used only for treatment purposes, in line with the privacy Policy below.

6. Liability

I strive to provide the highest quality care, and not liable for:

• Adverse outcomes due to incomplete or inaccurate patient information.

• Non-compliance with treatment plans.

7. Changes to Terms

I reserve the right to update these terms and conditions at any time. Changes will be communicated via website/email.

Any paper forms you complete or sign will be uploaded to your electronic record and then securely destroyed by a certified confidential waste disposal service.

Podiatry records are typically retained for a minimum of eight years from your last visit.
